I've been using RPG Maker 2003 for a long time now, and have never had a problem. A few months ago, I found that when I entered Test Play, the arrow keys were no longer moving the character or cursor.
I thought nothing of it since I wasn't really using the program at the time, but now I'm starting a new RPG and I kind of need it. I've tried uninstalling/reinstalling, as well as simply restarting the computer. I've found that Alt, 0 on the keypad, and most of the bottom row of letter all register as "cancel" or "accept", but nothing is mapped as arrow keys.

Anybody had a similar experience? Anyone have a solution?
.... Weird. Have you checked key input processing?
Is it just RM2K3? I have a similar problem with my computer, but it affects all the versions of RPGmaker (and nothing else).
If you have a joystick or pc gamepad plugged in, sometimes they can interfere with the keyboard input from rpgmaker. If you have one or both of these, try unplugging them and trying again.
kill ituneshelper process.
That helped me.
