Is there some sort of hidden hit/miss formula in the RM2K3 battle system? I've been testing battles using weapons with 95% and 90% hit probability and way too often I go on long miss streaks (missing 5 times in a row) with auto-attack.

Even if the hit probability is set to 100%, the attack may still miss if the enemy's agility is high.
If you want it not to matter, set everyone's Agi to 255. Including your party.
Also, auto-attack sucks. I tested my starting hero with it, and not only are there screwy glitches (it skips codes like attack command in the battle events, and sometimes casts guarded abilities like limit breaks), but yes attacks miss alot. There's no reason not to remove it. Flee sucks almost as bad, as there's no way to set any conditions for it, meaning you can block the Run Away option exp in commands if you're got a shared exp event, but you can't block exp from that (leaked exp always breaks).
I've never seen it skip code for attack command in battle events for me - and I use it in all my battles for all characters. And I don't really know what you mean by guarded abilities (limit breaks???).
So, assuming I remove it, any good replacement for a basic attack then? Other than going in the skill menu...

Basically, if there are special conditions for being able to use a skill, the auto-attack will ignore them and allow you to cast the skill anyway.

Actually, I think I found the reason why stuff kept missing; I decided to do a check to see if hero speed was really actually increased in battles. In one of the encounters I had it actually play the event for resetting speed back to normal twice (yeah, I only raise hero speed right before a fight, then set it back to normal right after). At least I now know that having low speed actually affects hit chance, that's pretty useful to know...
