I think he means it's something that WIP can do to your avatar (if he likes you enough? Idk either.)
Some of them are pretty weird though. I think I do remember Karsu actually having an "asspear" as an avatar at one time but that was a while ago but the phrase is still there.
I still love Geodude's for some reason, idk why.

I think it was AFrenchDreamer.
It is a complimentary gift for being nominated for a Misao award. Because you can no longer nominate games, you can no longer receive a user title.

No, you vote for games. Prior to the new Misao system, you had to nominate games before they were eligible for voting. Now all games are eligible for votes without need for nomination.
For a while, I was just tacking what I wanted it to say directly onto my avatar. Now I no longer care do so.
If you're desperate for a change of username colour and a witty one liner under your avatar and you're using firefox you could always write a Greasemonkey script to do it. The downside being that no-one else will see it because it modifies pages after they've been fetched from the server. Only you will know how cool you are.
