I think that something with a lasting impact (like a faceset change for the rest of the game) should be due to a weighty circumstance, unless changing costumes is a typical part of your game (like Persona 4's seasonal/athletic outfits). If you start off with a happy hero who suddenly gets half-scalped by the antagonist, I think that's certainly weighty enough and provides a juxtaposition that will stay with the player for a long time if done well.
