Hiroshi Tamawari's work sounds pretty great in my ears. That'd be old Vandal Hearts' and Azure Dreams' OSTs.
Also Miki Higashino's Suikoden 2 OST. Other than that, Suikoden 5, Breath of Fire 3, Grandia, Final Fantasy VII & VIII, and Wild Arms and Front Mission 3.
Front Mission 3 has excellent soundtrack.

Chrono Cross, without a doubt.
Honorable mentions are Chrono Trigger, Okami, Donkey Kong Country and Odin Sphere, give and take a handful of games I'm probably forgetting. The best soundtracks I've heard on portable systems are probably Dragon Warrior Monsters, Golden Sun and The World Ends With You.
Golden Sun had AWESOME music.
I'm really fond of the Crystal Chronicles soundtrack. It used a lot of interesting instruments like flutes, drums and trumpets.

All deserve a mention.
I'm not sure about Generals (I think that game sucked so I didn't play it for very long), but the other two are awesome. Along with the first Valkyrie Profile.
Also, I agree that Chrono Cross had great music, too. But the game itself was kinda shit.
I'm sticking by Nobuo Uematsu and Shoji Meguro.
I forgot to mention odin sphere as well. I think the music there was composed by Hitoshi Sakimoto as well.

I loved the theme for Fields of Fum.
@Mr Nemo: What? You didn't like the overworld song of the original game? That's one of my favorite overworld themes, ESPECIALLY for handheld!
@Karsuman: Y'know, CC wasn't perfect, but the music of that game was nearly perfect. I have that CD set. I played that game through three times on account that I wanted to hear the music of it. But it's true. There were so many characters and so little development of them that it felt so... bleh. I still like Razzly the Fairy, though, she's cute.

Wild Arms' music wasn't all that great to me (much like the game itself). Wild Arms 2 was better at everything, especially the music.
I always think it is interesting that nobody ever mentions bad games with good music. I am pretty sure it is the game that you like, and not the music.

Well, I've never played Soul Hackers, but the soundtrack is great. I doubt it's something most people could get into, though.
