Also killing someone for their incompetence as a healer is a bit extreme. Why they could have just escorted the guy back to the nearest village and told him that he was no longer needed in the party?

I think the problem is that the scene that plays when you exit the cave tells you "go back, you will not find anything outside"
The same happened when I played, died in the cave, but I restarted and found the rest of the game...
I think the dialogue could do with a bit of tightening up as a few people seem to have got a bit confused about where they need to go.
I feel I should say however that no-one says "you will not find anything outside" or words to that effect. Henriet says they have gone the right way and suggests they should push on. Tybalt and Skidds say they should go back into the cave because there might be treasure.

Skidds' motivation for murdering Vinae is explained in the introduction - due to Vinae's incompetence the party have had to use all their recovery items just to keep him alive so Skidds decides they are better off without him.
