Yes there are a ton of NPCs in the first city and lots of entirely optional content! :)
You're so right about the tone, it mixes dark and light a lot, and I do think the contrast is nice for the player without being jarring.
Yes there are 4 settings, so there's a difficulty for everyone, be you casual, challenge lover, or just plain nutcase. ;)
