You may have an old version of the game ogswan. There was a bug with that chest. If you download the newest version and put your save into the folder of the downloaded file the chest should appear at the entrance of the tower. (I think it contained a shield)
Also there is no big reward for getting all those chests iirc.

For Boss #3 not sure if it's a bug or intended:

In Hades' Maw the tablet that teleports you to the last room, seems to do this: fadeout screen, teleport, NOT fadein screen, so the whole screen is black after you get teleported to hades. I had to try for a good 5 minutes to find the Hades fight by randomly moving around. During the fight itself the screen is also fully black. Now, maybe it was intended? Since Hades is like the king of the underworld and stuff, so darkness fits :P
Edit: Well I just killed Hades and somehow got back to the world map (at least the world map music is playing) but the screen is still black
