A chest in Section 3 of the Cave of Surt gives Steady Shoes, while the message says that you received a Berserk Ring. Looking back over the maps, there's also a chest in the 'new' section 7 of the Water Shrine that you receive a Practice Spear when it says you should have gotten a Wooden Spear.
I'm currently not sure if there are any other instances where you receive one thing when you should have(?) received another, but it's something to look into.

I just want to echo a piece of what Kalledemos stated in that, once you fight Orphan, it doesn't give you the trophy for it (despite there being a space for it in the Rift Shrine). What seems to have happened is that you didn't turn that one switch that acknowledges you fought Orphan on.

While exploring the Cave of Memories, I found a pass-ability issue with the crisscrossing bridges that prevents the player from progressing.

Is there some way out of the Trei Cave if you return after saving Maria from the sahagin but before clearing the shortcut between it and the Trei Keep as Hans? I tried to backtrack and I got stuck at the first treasure chest pushing puzzle.
Luckily, I had two save files, one of which I had saved before finding myself trapped.
