I agree with Psy, categories needs to be revisited next year. I love most RTP game, but it doesn't work in a popular choice contest. It would work in something more educated, tho.
So, categories can be very specific, but in that case nominations need to be made by a jury and only votes left for popular choice. I don't think it's a good solution.
The other option is to reduce number of categories and give them more prestige (a badge on gamepage would be nice for example).
Personally I find it easy to nominate games in following categories:

Best graphics
Best setting/story/characters (I would make that one category. Something like excellence in narrative)
Trippiest game
Best game title
Technical provess
Best level design
Best fan game
Best demo
Best game

This is my first time actually following the Misao's. Are they usually this...tiny? Only a handful of games were nominated for anything.

Dumb Castle may be made out of 100% RTP assets, but, best use of those assets? I dunno. However, yeah, it's nowhere near popular enough to receive a Misao.

*Edit: I dunno about displaying anything on a gamepage like Featured Games have. Games can (and maybe should) tout that they won game of the year in whatever year, but, more than that?

in my experience, anything other than the light and airy popular vote contest we have now (Misaos) is fraught with difficulty, animosity, hurt feelings, disdain, hubris, scorn, condescension, anger, and superciliousness, among other things. People get really uptight when it comes to any sort of official recognition and ranking.

I wasn't around in 2008, so maybe this suggestion is off base, but what about a combination of the two systems?

Games could be nominated in different categories by popular vote and then a jury selects winners from among those nominations. It ensures that users still have a voice, but keeps something ridiculous, like Umbral Soul winning best fangame, from happening.

Nope, that doesn't work. It would conclude in the saltiest result imaginable.
It could work the other way around, tho. A jury would choose nominees and people vote for them. Popular choices could be added.
Still I find misaos a pure popularity contest as the best possible option. The only thing, which needs to be done is to make it more visible (and that's quite difficult) and to have less categories. Having 6-12 categories would make each individual award more important.
And I basically agree with Marrend that a gamepage badge isn't such a good idea. Maybe misaes would get more nominations, if creators of popular games were noticed about a possibility to urge their fans to vote for their game. I guess people don't like promoting their games in this way as it may look cocky, but over 4000 people played Umbral and only some 20 voted (It's game with 9 reviews btw).
And even wheelman would like to see more people supporting his game with this little gesture by my reckoning.
