It was one of the random step-encounter battles in the caves below the city. I believe it occurred on one of the maps to the south of the map with the treasure chest on the raised wall.

I'm not sure if it's a scripting error with the battle system, but whenever the main character dies, the battle freezes but still shows the female hero walking.

About "some huge design flaw", you could always beta test your own game during the process to make sure that no bugs remain when it comes time to release a demo.
EDIT: It might just be my computer, but I got two seconds into the game (font is missing?) and I get an error that says "Script 'Bitmap, Draw Face 1.5' line 18: RGSSError occured. failed to create bitmap"

Sorry that people didn't kiss your ass enough to make it worth even your time to play... Honestly, such childish behavior is best suited for playing MMORPGs.

A game of tag?
I've decided that if the player is not it and stands still for two seconds, the game ends. You win if you can keep from getting tagged for a minute. I've also gone through and cut out the unnecessary sections of coding by using labels and increased the ways that NPCs can tag each other from the previous version that I had realized only works on two directions to eight directions.
Also, the game keeps count of how many games of tag you've played that may be used later for achievements and unlockable content.

Honestly, though, it won't be that much more work. I just need to improve what I have and enhance my plot/dungeon balance in the second half of the game to keep everyone playing. Also, Ashriot will be working on our tactical battle system for Carlsev 2, so I can't do much for the next few weeks regardless :)
I've been doing the same thing on DoaDV for the past 4 years. Trust me, improving is great if you can get a final product rolled out after the first revision, but if you're someone like me who works on one section, moves onto another, and has seen such a grand amount of improvement in such a small amount of time that he feels forced to redo what he just got done redoing, it snowballs.
