Gimme Boss Gimmicks
I suppose "Any" is too strong of a word. If there's a valid reason for the transformation/powerup, I'm game. For example, using the were-creature example, if you're fighting a lycanthrope in human form and they don't want the fact that they're a lycanthrope to be revealed, I can see them fighting in human form at first until they get more desperate and transform, revealing his secret.
Now if the same lycanthrope was found in his secret hideout, he's planning on killing the party, and the and there's no one to hide from, why not have the lycanthrope start off transformed? When the fight starts with the lycanthrope in human form who changes during the fight, that's the shit I hate (unless there's another reason, like maybe the more the lycanthrope's in his transformed state, the more insane he is or something)
When the reasoning behind the transformation aren't stupid (ripped from DBZ, there just to have a transforming boss, SO YOU REVEAL YOUR TRUE FORM (Not complaining about SF64 here), ect.), that's what I hate. I'm probably just grouchy from fighting the Morgan. That's a 'transforming' boss that just plain stupid. Twelve missiles later and he finally turns on his anti-missile shield and activates the time limit of doom? What was he doing for the last couple minutes?

I personally hate any sort of transformation gimmick. It reeks of "That was only a fraction of my power. Now I'll show you my true power/suffering/poorwritingskills!" It hurts my brain and it gives me another reason to quit playing (doubly so if there some stupid villian monologue while doing so, the process takes five minutes, and I don't get a chance to use items since the party had their heads stuck up their own asses during the intermission)
Yes, this makes me a hypocrite; I've got a transforming final boss for DG. I've got a bit of ingame explanation to why, but at the end of the day its the same shit. I hate transforming bosses with silly explantions.

I'd bet its a folder with a video in it, Explorer is trying to generate a thumbnail of it, fails, and crashes.
Gimme Dungeon Gimmicks
Water areas should be supplemented by this picture:

If you've played La Mulana, you've probably got yourself in this situation.
Here's the jist of it:
The player has just done some annoying jumping puzzles (and avoiding Surprise! Fish's) to finally be able to swim in water without taking damage. (Note: You have to swim through water to even get close to where the item is)
So now the player can swim and they can start exploring the area further. Now is a good time to look for where ROMs are, most of which are invisible although there's some idea where they are (A difficult platform to jump to? There's likely a ROM there even if you can't see it). So the player goes to this room, sees the lower right corner which is interesting, and decides to check it out. Now the wall next to the player isn't there so you can enter it, and the blue wall is impassable. See where its going?
The player goes to investigate the corner and suddenly gets trapped there!
Further kick in the teeth: With a special item (that you should get long before you get here), the player can teleport back home. Getting trapped really isn't that bad, all you do is open the menu and push a button 0-9 and teleport to that area (if you've unlocked it, 0 is always unlocked). BUT you can't open your menu in water without another special item! You're forced to quit the game, reload, and do all that again!

Damn, I vaguely remember that game! No idea what its called though. Starts out with little demons running around that don't do anything to hurt the player I think.

The ITEM! font looks horrendous. Do something similar to what you did with the item name/location because it sticks out like a sore thumb.
The exclamation marks are also off in their own little world. Can you shimmy them over a bit so they're closer to the end of the phrase?
