I didn't expect to click on this topic and be subjected to reading about menstruation in RPGs
This is a horrid approach to game design. If it doesn't add anything to your game, there's no point. For example - yeah, day/night cycles can be cool, but if I randomly decided to add them because I can it would be pointless because they'd serve no purpose. Likewise, adding a menstruation mechanic wouldn't only be pointless, it would actually take away from my game because the idea is so utterly absurd and quite sexist (and really, would just be annoying)

extremely hard and especially difficult games?
My game I released back in 2008, Hellucination, is ridiculously hard. I was actually working on a new version shortly after its release that fixed bugs/made it far easier/added content/modified the storyline and dialogue, but it was on an old hard drive that I lost.

At the end of my game, things start to become more surreal and bizarre. This is the beginning of the transition. As always, my focus is on atmosphere. The atmosphere here is supposed to be somewhat foreboding and uncomfortable, but also strange.
My game is nearing completion. If you're interested in a 10+ hour RPG with tons of side quests, lots of character development, an extremely vivid and well crafted world, and an interesting story, I suggest you subscribe to my game. I've been having trouble generating hype for it. Marketing isn't exactly my strong point - my focus has been on making an excellent, well rounded, interesting game. Here's a link:

The main issue is that all those fine details - the vines, trees, etc - should be placed more randomly. Right now, the vines are placed in same pattern across the walls. That would never happen in nature.

Thinking about the 'mysteris of RPGMaker'...
A couple years ago, I worked with someone (I can't recall who...sorry!) and we got RM drawing arbitrary graphics primitives (mostly lines) of any color.
I was shocked that nobody cared. Apparently, nobody ever wants to draw shapes using RPGMaker :(
I don't understand what you mean, but it sounds interesting. Can you explain more?
